This form is for ordering from the four STANDARD house signs shown below:

1.  Choose the shape of your sign (print out & circle one):
                     
2.  Background color (circle one): Hunter Green, Royal Blue, Deep Red, Colonial Blue

     Also choose an accent color (circle one): Beige, White, Cream.

4. Sides to be carved (circle one): Single Sided, Double Sided.

5. Figuring your cost:  $35.00 per letter (approx. 2 1/2” high letters, carved & gold  leafed.)
                              There is a $75.00 minimum charge.
                              Add $60.00 if you want the sign edge gold leafed.
                              Shipping and handling is approximately $15.00.
                              PA residents add 6% for sales tax.

6.  Payment:  50% is due with your order (check or money order).  The remainder is due when you receive your sign (COD).

7. Delivery time:  Because our signs are all hand crafted, they do take time to make.  Please allow approximately 8 to 10 weeks for delivery.
